When keeping the tooth is not the better option

Tooth Removal in Mapperley, Nottingham

Removing a tooth that cannot be saved, or that is causing recurring problems, with clear aftercare and a plan for the gap.

Extraction is a last resort for most teeth and a first-line option for some wisdom teeth. Either way the decision is made after assessment, and what happens to the space is planned before the tooth comes out.

The straight answer

What does tooth removal involve?

Tooth removal is the extraction of a tooth that cannot be restored or is causing recurring problems. The area is numbed with local anaesthetic, the tooth is loosened within its socket and lifted out, and the socket is cleaned. Some teeth, particularly impacted wisdom teeth, require a surgical approach and stitches.

When it is recommended

Teeth that are broken below the gum, split, extensively decayed beyond restoration, or loose from advanced gum disease. Wisdom teeth are removed where they cause repeated infection, damage the tooth in front, or are associated with cysts, rather than simply because they are impacted.

How it works

Local anaesthetic numbs the tooth and surrounding tissue. Instruments widen the socket and break the fibres holding the root before the tooth is lifted out. Surgical extraction involves lifting the gum and sometimes removing a small amount of bone or sectioning the tooth.

What happens next

A blood clot forms in the socket and gum tissue closes over the following weeks, with bone filling in over months. Where a gap will be visible or affect function, replacement options are discussed before the tooth is removed.

Why patients come to us

When extraction is discussed

01

"My wisdom tooth keeps getting infected."

A partially erupted wisdom tooth can trap food and bacteria under a flap of gum, causing repeated painful infections that antibiotics settle but do not prevent.

Practical outcome: removing the tooth removes the trap, so the infections stop recurring.

02

"My tooth has broken off at the gumline."

Where too little sound structure remains above the gum, there is nothing to build a crown onto, and attempting to restore it is unlikely to hold.

Practical outcome: an honest assessment of restorability, and a plan for the space before the tooth comes out.

03

"A tooth has become loose."

Mobility usually indicates advanced loss of the bone supporting the tooth. Where too much support has gone, the tooth cannot be stabilised.

Practical outcome: the tooth is removed before it causes an abscess, and gum treatment protects the remaining teeth.

Potential benefits and things to consider

What this treatment gives you, and the practicalities worth knowing before you book.

Potential benefits

  • Resolves recurring infection. Removing a problem wisdom tooth stops the cycle of repeated infections around it.
  • Removes the source of pain. An unrestorable tooth cannot be made comfortable any other way.
  • Protects neighbouring teeth. A wisdom tooth damaging the tooth in front, or a tooth with severe infection, can affect its neighbours if left.
  • Usually a single appointment. Most extractions are completed in one visit under local anaesthetic.
  • Enables the next step. Removal is often the first stage of a plan involving a denture, bridge or implant.

Things to consider

  • It is irreversible. Once a tooth is removed it cannot be put back, which is why restorability is assessed carefully first.
  • Bone shrinks afterwards. The ridge of bone that held the tooth resorbs over the following months, which affects later implant or denture planning.
  • Neighbouring teeth can move. Teeth either side can drift into a gap and the opposing tooth can over-erupt.
  • Replacement costs more. An implant, bridge or denture to fill the space is a separate and usually larger cost.
  • Some cases need referral. Deeply impacted wisdom teeth close to the nerve are often better managed by an oral surgeon.

Suitability

When is tooth removal the right option?

Typically a good fit

  • Teeth broken or decayed beyond predictable restoration
  • Wisdom teeth causing repeated infection or damaging the tooth in front
  • Teeth loose from advanced gum disease
  • Teeth being removed as a planned part of denture or orthodontic treatment

Where more assessment is needed first

  • Medication affecting bone healing, including bisphosphonates and some cancer therapies
  • Blood-thinning medication, which may require planning but rarely prevents extraction
  • Impacted wisdom teeth lying close to the nerve in the lower jaw
  • Recent radiotherapy to the head or neck
  • Uncontrolled medical conditions, which may need addressing first

When we may suggest something else

  • Root canal treatment and a crown, where the tooth is restorable
  • Gum treatment, where mobility is due to inflammation that can be stabilised
  • Leaving an asymptomatic impacted wisdom tooth in place under review
  • Referral to an oral surgeon for surgically complex cases

What we ask of you

Attend the examination and hygiene intervals your dentist recommends

Follow the home cleaning routine agreed with your dentist or hygienist

Tell us about medical conditions and medication, and about any changes

Your dentist will confirm whether this treatment is suitable after examining your teeth, gums, bite and wider oral health.

Step by step

What happens when a tooth is removed

01

Assessment and X-ray

The tooth, its roots and its relationship to nearby structures are assessed radiographically. Restorability, alternatives and the plan for the space are discussed before anything is agreed.

02

Consent and planning

Risks, aftercare and replacement options are explained. Medical history and medication are reviewed, particularly anticoagulants and bone-affecting drugs.

03

Local anaesthetic

The tooth and surrounding tissue are fully numbed. You should feel pressure and movement but nothing sharp.

04

Removing the tooth

The socket is gently widened and the tooth lifted out. Surgical cases involve lifting the gum and sometimes sectioning the tooth so it can be removed in pieces.

05

Settling the socket

The socket is cleaned, bleeding is controlled with pressure, and stitches are placed where needed. You bite on gauze until a stable clot has formed.

06

Aftercare and review

Written aftercare instructions are given. Any stitches and the plan for replacing the tooth are reviewed at a follow-up appointment.

The first appointment is an examination and a discussion. No treatment is carried out without your agreement, and you leave with a written plan and costs.

Comfort and aftercare

What treatment actually feels like

During treatment

The area is fully numbed with local anaesthetic. You will feel firm pressure and movement, which is normal and is not the same as pain. Tell your dentist immediately if you feel anything sharp.

Surgical extractions take longer and involve more pressure. Agree a hand signal before starting so you can pause at any point.

After treatment

Bite firmly on the gauze provided for at least twenty minutes. Do not rinse, spit, smoke or drink through a straw for the first twenty-four hours, since disturbing the clot is the main cause of a painful dry socket. Avoid alcohol and hot food that day.

Some bleeding, swelling and discomfort over the first two to three days is expected. Take painkillers as advised, keep the area clean with warm salt water rinses from the day after, and eat soft food on the other side.

Looking after your result

  • Attend the follow-up appointment so healing and any stitches can be checked.
  • Discuss replacing the tooth before the bone has resorbed significantly, since timing affects implant and denture planning.
  • Keep the remaining teeth and gums healthy, since the reason one tooth was lost often applies to others.
  • Contact the practice if severe pain begins two to four days after the extraction, which can indicate a dry socket and is treatable.

Common questions

Frequently asked questions about tooth removal

Do all wisdom teeth need removing?

No. Wisdom teeth are removed where they cause repeated infection, damage the tooth in front, are associated with a cyst, or cannot be kept clean. An impacted wisdom tooth causing no problems is usually monitored rather than removed. Your dentist will assess the specific tooth radiographically before recommending anything.

Will it hurt?

The area is fully numbed with local anaesthetic, so you should feel firm pressure and movement but nothing sharp. Discomfort afterwards is common for two to three days once the anaesthetic wears off and is usually managed with ordinary painkillers. We cannot promise a pain-free experience, but we can stop and add more anaesthetic at any point.

How long does healing take?

The gum closes over the socket within two to three weeks and the underlying bone fills in gradually over several months. Most people feel substantially better within three to five days. Swelling peaks around day two or three after a surgical extraction and then improves.

What is a dry socket?

A dry socket occurs when the blood clot protecting the socket is lost or fails to form, leaving bone exposed. It typically causes severe pain starting two to four days after the extraction, often with a bad taste. It is treatable at the practice with a dressing, so contact us rather than waiting it out.

Do I need to replace the tooth?

Not always. Back teeth that are not visible and do not affect your bite are sometimes left. Where the gap is visible, affects chewing, or would allow neighbouring teeth to drift and the opposing tooth to over-erupt, replacement with an implant, bridge or denture is worth planning before the extraction rather than afterwards.

Can I drive myself home?

Yes, after an extraction under local anaesthetic. You will be numb but not sedated. Arrange transport if you are having sedation, which would be discussed and organised in advance rather than on the day.

I take blood thinners. Can I still have a tooth out?

Usually yes. Most patients on anticoagulant medication can have teeth removed safely with appropriate planning and local measures to control bleeding. Do not stop any medication without advice from the prescribing doctor. Tell the practice about all medication when booking so the appointment can be planned properly.

I took my 7-year-old son to this dentist for the first time and was really impressed with the experience. The decor was tranquil, calming and aesthetically pleasing, which immediately created a lovely atmosphere. The staff were welcoming, friendly and knowledgeable, and the dentist was particularly helpful in making my son feel comfortable and relaxed. He had his teeth cleaned with absolutely no issues — he didn’t cry or show any signs of discomfort, which was a huge relief as a parent. Based on our first impression and experience, I would definitely recommend this dentist, especially for parents looking for a welcoming and child-friendly practice. We were very happy with our visit! Mapperley Dental Studios are exceptional! From Dr. Tamang, Mitu, Zoe and Dani, everyone is incredibly friendly, helpful and kind. I was recommended by my husband, and I have never regretted making that appointment. I was in a lot of pain due to a gum infection, and as someone with very sensitive teeth who is also quite nervous about going to the dentist, I was feeling very anxious. Dr. Tamang was incredibly gentle and took the time to explain everything to me, which made me feel comfortable and reassured. Zoe has always been there to hold my hand and make me feel comfortable throughout my treatment. I am now a regular patient and will definitely continue coming back for my dental treatment. Thank you to the whole team for making my experience such a positive one!

Amazing! Since my first appointment, the whole team have been great. Like many people, I’m not a fan of going to the dentist, but they were incredibly patient and attentive and made sure I was comfortable at all times. Time was taken at every appointment and I never felt they were in a rush, which I have felt at other practices. Made the whole experience so much easier. Thanks so much!

These guys definitely deserve 5*! The place is lovely, everyone is very friendly and professional, and they made me feel completely at ease every time. I have a bit of a “white coat syndrome” 😊, so going to the dentist isn’t exactly my idea of a relaxing day out! But everyone here is so kind, reassuring, and patient that every visit so far has been a pleasant experience. Highly recommend!

Start with a consultation, not a commitment

Your first appointment is an assessment and a conversation. You leave with a written plan setting out what is recommended, in what order, and what it costs, and you decide what happens next.
