What is clear aligner teeth straightening?

Clear aligner treatment straightens teeth using a series of custom-made, removable transparent trays. Each aligner is worn for a set period and moves the teeth a small amount before being replaced by the next in the sequence. Because the trays are removable, you take them out to eat and to clean your teeth normally.

The problem it treats

Crowding, spacing, rotated teeth and certain bite problems. Crowded teeth are harder to clean and more prone to decay and gum disease, so alignment is not purely a cosmetic issue.

How it works

Digital scans of your teeth are used to plan the movements. Each aligner is slightly different from your current tooth position, applying gentle sustained pressure. Small tooth-coloured attachments may be bonded to some teeth to give the aligner grip for particular movements.

What is realistic

Aligners work only while they are in your mouth. The standard expectation is 20 to 22 hours a day. Teeth also have a strong tendency to move back, so retainers are required indefinitely afterwards to hold the result.

What happens during aligner treatment

Assessment

Your teeth, gums and bite are examined and any decay or gum disease is treated before orthodontic movement is considered.

Records and digital planning

Digital scans, photographs and X-rays are taken and used to plan the sequence of movements. You are shown what the plan intends to achieve.

Fitting the first aligners

Any attachments needed are bonded to the teeth, the first aligners are fitted and checked, and you are shown how to insert, remove and clean them.

Progressing through the series

You change to the next aligner at the interval your dentist specifies. Tightness and mild tenderness for a day or two after each change is normal and indicates the aligner is working.

Reviews

Progress is checked at intervals so that any tooth not tracking as planned is identified early rather than at the end.

Refinement and finishing

Additional aligners are commonly needed to finish the detail. Attachments are removed and edges are polished.

Retention

Retainers are fitted immediately. These must be worn as instructed indefinitely, because teeth move back if they are not.

Comfort and aftercare

What treatment actually feels like

During treatment

Fitting aligners is not painful and requires no anaesthetic. Bonding attachments involves no drilling of healthy tooth.

Each new aligner feels tight for the first day or two and teeth can be tender to bite on. This is expected and settles. A speech lisp for the first few days is common and almost always resolves.

After treatment

Remove aligners to eat and to drink anything other than water, since hot drinks distort them and sugary or coloured drinks sit against the teeth beneath.

Clean the aligners and your teeth every time they go back in. Trapping plaque under a tray for 22 hours a day causes decay and gum inflammation quickly.

Looking after your result

  • Wear your retainers exactly as instructed, indefinitely. This is the single thing that determines whether the result lasts.
  • Bring retainers to your routine appointments so their fit can be checked.
  • Replace retainers when they wear out rather than going without.
  • Keep up examinations and hygiene visits throughout and after treatment.

Frequently asked questions about teeth straightening

How long does clear aligner treatment take?

It depends entirely on how much movement is needed. Minor front tooth crowding can take a few months; more extensive cases take longer. Your dentist will give you an estimated number of aligners and a timescale once your case has been planned digitally, and that estimate assumes you wear them as instructed.

How many hours a day do I have to wear them?

The usual instruction is 20 to 22 hours a day, meaning they come out only to eat and to clean your teeth. Aligners apply pressure only while they are in place, so reduced wear does not slow treatment down proportionally, it stalls it and often means teeth stop tracking to the plan.

Are they really invisible?

They are transparent and discreet rather than genuinely invisible. Most people will not notice them in normal conversation. The small tooth-coloured attachments bonded to some teeth are visible close up, and they are necessary for certain movements to work.

Does it hurt?

There is no drilling and no injection. Each new aligner feels tight for a day or two and teeth can be tender to bite on, which is the movement happening. Most people manage with ordinary painkillers for the first couple of days of each change, and many need nothing at all.

Will my teeth move back afterwards?

They will, unless you wear retainers. Teeth have a strong tendency to return towards their original position, and this does not stop after a year or two. Retention is a permanent commitment, and retainers eventually wear out and need replacing. Treat this as part of the cost, not an optional extra.

Can I whiten my teeth at the same time?

Whitening is normally carried out after alignment rather than during it, because moving teeth exposes surfaces that were previously overlapping. Whitening at the end means the whole visible surface is treated evenly. Your dentist will sequence this in the written plan.

Can aligners fix any problem?

No. They are well suited to mild and moderate crowding, spacing and rotation. Significant bite discrepancies, large movements and complex cases may need fixed braces or a specialist orthodontist. An honest assessment of whether aligners can achieve what you want comes before treatment is offered.

Mapperley Dental Studios are exceptional! From Dr. Tamang, Mitu, Zoe and Dani, everyone is incredibly friendly, helpful and kind. I was recommended by my husband, and I have never regretted making that appointment.
